Wandsworth End of Life Care Coordination Centre

The centre will be open from 7am until 8pm Monday-Friday.
We can order and authorise equipment requests
We can deliver care coordination (e.g. chasing up medication from a GP, liaising with DN team)
We can do fast-track Continuing Health Care referrals to approved care providers experienced in EOLC
We can arrange other services e.g. volunteer befriending

 

By late February, we will have a fuller cohort of staff and can extend our hours into the evening (until 10.30pm) and weekends (9-5) as planned. By this stage we will also be managing referrals into the Marie Curie night nursing service.
